Express your wishes regarding medical treatment by detailing who will make decisions on your behalf when you are unable to do so yourself. If you are incapacitated and cannot communicate your care instructions directly, advance directives and powers of attorneys are your voice to doctors and caregivers and to the trusted persons whom you have appointed to act on your behalf.
